Team Waiver, Code of Conduct, and Media Release Agreement *

Please read carefully before agreeing: By checking the box below and completing my (or my team’s) registration, I acknowledge that: - I voluntarily assume all risks of participation in the 2025 KAIDAP Summer Basketball League ("the League") and release KAIDAP, its staff, organizers, facility owners, sponsors, and partners ("Organizers") from any and all liability for injuries, losses, or damages, including those caused by negligence. - I understand that participation involves physical exertion and possible risk of serious injury or death, and I confirm that I and all participants on my team are physically fit to participate. - I agree, on behalf of myself and my team members, to behave respectfully, demonstrate sportsmanship, and follow all rules and instructions from League staff and referees. - I accept that the Organizers have full discretion to interpret inappropriate behavior and may remove any player or entire team from the League without refund if behavior is deemed inappropriate, unsafe, or detrimental to the League environment. - I understand and agree that all payments are non-refundable, except in the case that the League is fully canceled by the Organizers. - I grant permission to the Organizers to use photographs, video recordings, and/or audio recordings taken during League activities — which may include the image, voice, or likeness of myself and my team members — for promotional, advertising, or informational purposes in print, digital, or social media formats, without compensation or additional consent. - I acknowledge that by agreeing to these terms, I am confirming acceptance of this Waiver, Code of Conduct, and Media Release on behalf of myself and all players registered under my team, and that it is binding for each individual participant on the team.
